Payment Information

Services are billed monthly accordingly to your preset schedule.
Service fees paid by agency benefits will follow that agency's
guidelines. Payment is due upon receipt. A 5% late charge will
apply after 10 days. Non payment of services after 30 days
will result in termination of membership.
